Background: The characteristics of synovial fluid (SF) in geriatric patients differ from those in younger patients. In Mexico, epidemiologic data on the incidence of different rheumatic diseases in geriatric patients are scarce. Objective: To describe the physical characteristics of geriatric SF and the prevalence of crystals in knee and other joint aspirates from patients with previously diagnosed joint disease. Materials and methods: A retrospective study was performed with a baseline of 517 SF samples between 2011 and 2023. White blood cell count was performed by Neubauer chamber and crystals were identified by polarized light microscopy. Descriptive statistical analysis was performed and prevalence was reported as a percentage. Results: The mean age of the adults was 73.5±5.0 years, 54.4% were women and 45.6% were men. The mean SF volume was 6.3±9.5mL in older adults and 15.3±24.9mL in those younger than 65 years. The mean viscosity in older adults was 9.5±4.5mm and the mean leukocyte count was 7352±16,402leukocytes/mm3. Seventy percent of the older adults’ SFs were referred to the laboratory for osteoarthritis (OA), with lower proportions for rheumatoid arthritis (RA) (14.6%) and gout (5.1%). Of the crystals observed in the geriatric population, 14.6% corresponded to monosodium urate crystals (CUM) and 18.9% to calcium pyrophosphate crystals (CPP). Conclusions: The characteristics of LS in older adults were smaller volume, increased viscosity, and non-inflammatory. The main diagnoses were OA, RA, and gout. The crystal content of the SF of the geriatric population corresponded mainly to CPP.(AU)


Antecedentes: Las características del líquido sinovial (LS) en pacientes geriátricos varían en comparación con pacientes más jóvenes. En México, los datos epidemiológicos sobre la incidencia de diversas enfermedades reumáticas en el paciente geriátrico son escasos. Objetivo: Describir las características físicas del LS geriátrico y la prevalencia de cristales en aspirados de rodilla y otras articulaciones de pacientes con enfermedades articulares previamente diagnosticadas.Materiales y métodos: Se realizó un estudio retrospectivo con una base de 517 muestras de LS entre 2011 y 2023. El recuento de glóbulos blancos se realizó con cámara de Neubauer, y los cristales se identificaron por microscopia de luz polarizada. Se realizó un análisis estadístico descriptivo y la prevalencia se reportó como porcentaje. Resultados: La edad promedio en los adultos fue de 73,5±5,0 años; el 54,4% fueron mujeres y el 45,6%, hombres. El volumen promedio del LS en adultos mayores fue de 6,3±9,5ml, mientras que en menores de 65 años fue de 15,3±24,9ml. La viscosidad promedio fue de 9,5±4,5mm en los adultos mayores, y una cuenta de 7.352±16.402 leucocitos/mm3. El 70% de los LS de los adultos mayores fueron remitidos a laboratorio por osteoartritis (OA), u una proporción más baja, por artritis reumatoide (AR) (14,6%) y gota (5,1%). En cuanto a los cristales observados en los LS de la población geriátrica, el 14,6% correspondieron a cristales de urato monosódico (CUM) y el 18,9%, a cristales de pirofosfato de calcio (CPP). Conclusiones: Las características del LS en los adultos mayores fueron menor volumen, viscosidad incrementada y no inflamatorios. Los principales diagnósticos fueron OA, AR y gota. El contenido de los cristales en los LS de la población geriátrica correspondió principalmente a CPP.(AU)

Osteoarthritis is a disabling pathology characterized by joint pain and stiffness. A prevalence of coxarthrosis of 7.4% is reported in our country. Total hip joint replacement is indicated in advanced stages, a procedure that is not free of complications, the most frequent being prosthetic dislocation, which can be prevented with dual mobility systems. The following study aims to determine the rate of complications and clinical outcomes in dual mobility systems in primary coxarthrosis. MATERIALS AND METHODS: A retrospective study included 120 cases in 114 patients diagnosed with grade III coxarthrosis, mean age was 62.43 years, with a mean follow-up of 4.5 years. Joint replacement was performed by Hardinge approach. All cases were assessed clinically using the Harris Hip Score (HHS) and radiologically to demonstrate mid-term results. RESULTS: The preoperative value on the HHS scale had a mean of 56.45, postoperative at one month 74.23; 6 months 85.40; 1 year 94.01 and at 5 years 94.84 points, representing a functional improvement of 17.78 postoperative month; 28.95 at 6 months postoperative; 37.56 at one year postoperative and 38.39 points at 5 years postoperative. A complication rate of 3.44%; 0.86% of complications were associated with the prosthetic components. CONCLUSION: The dual mobility system should be considered as a therapeutic option in primary hip joint replacement due to excellent functional results and low complication rates. EVIDENCE LEVEL: IV. INTRODUCTION: Closing wedge high tibial osteotomy (CW-HTO) is a surgical option for active patients with medial knee pain and mild-moderate osteoarthritis with varus limb deformity. Despite its good reported results, this technique has been losing popularity. The aim of this study was to analyse the survival rate, clinical functional outcomes and radiological results of CW-HTO. METHODS: It is a retrospective case series study. Seventy patients with primary knee osteoarthritis, operated on between 2010 and 2020 in a single Spanish tertiary hospital using the CW-HTO technique and with a minimum follow-up of 2 years were analysed. RESULTS: Survival rate was 87.6% and 75.5% after a follow-up of 5 and 10 years respectively. Functional outcomes were good-to-excellent (KSS 77.7/100 and OKS 35.6/48) and good pain control (VAS 3.9/10) and high satisfaction (7.2/10) were achieved. Limb varus malalignment was significantly corrected (mean postoperative HKA angle 177.6° and MPTA 90.7°). However, 30% of patients presented hypocorrection, which was associated with inferior survival, functionality and satisfaction. CONCLUSION: CW-HTO technique can be useful for patients with knee osteoarthritis and varus limb. It allows to correct varus malalignment while achieving good-to-excellent functional outcomes, good pain control, high patient satisfaction and acceptable medium-long term survival rate. However, it is associated with a non-negligible risk of hypocorrection or medial hinge disruption.

INTRODUCTION: Developmental dysplasia of the hip is the cause of approximately one third of secondary coxarthrosis. Anatomy alterations make it difficult to place a total hip prosthesis in its anatomical position and for it to be stable in the long term; there are several techniques to achieve this goal. In the present work, we used autograft of the femoral head (shelf graft or reinforced roof), to improve the coverage of the acetabular component with favourable results. MATERIALS AND METHODS: Sixteen cases were included in 14 patients with a diagnosis of developmental dysplasia of the hip (13 women and one man), the mean age was 44.3 years (range 35-68 years), with a mean follow-up of 7 years (range 1-15 years). All the cases were evaluated clinically and radiographically, to demonstrate the osseointegration of the graft and the functional results in the medium term. RESULTS: All the acetabular components were placed in anatomical position (Ranawat technique), the mean percentage of host bone coverage was 54.53% (range 43.28-79.05%), obtaining additional coverage with the bone graft of 45.13%. Osseointegration of the graft of 100% at 12 weeks, resorption of the graft from the sixth month, stabilising in the third postoperative year. Only one case of dislocation is reported, no cases of infection, loosening, heterotopic ossification or revision. CONCLUSION: This procedure has shown good functional results in the medium term with 100% osseointegration, despite cases of severe bone resorption of the graft that does not compromise the stability of the prosthesis.

ABSTRACT Introduction: Osteoarthritis (OA) is a chronic inflammatory degenerative disease, characterized by progressive degeneration of the articular cartilage, affecting mostly the knee joint. The elderly population is the most affected, intensifying the aging process. The concentration of intramuscular adipose tissue in the thigh muscles and knee OA at different ages remains under investigation. Objective: Investigate the cross-sectional area of thigh muscles at different ages with the relationship of the presence of intramuscular fat and knee OA. Methods: 80 participants were paired into 4 groups: Young Osteoarthritis Group and Old Osteoarthritis Group, both sedentary with knee OA diagnosis II or III; Healthy Young Group and Healthy Old Group, both healthy and sedentary. The groups were paired according to gender and body mass index, submitted to physiotherapeutic evaluation, WOMAC questionnaire, knee X-ray and thigh CT scans. Morphometric analysis was performed manually with ITK-SNAP software (version 3.6), by a single evaluator. Statistical analysis used the one-way ANOVA test followed by Bonferroni post-hoc for dependent variables (p≤0.05). Results: Comparison of the cross-sectional area of the rectus femoris, vastus lateralis, semitendinosus, sartorius and gracilis muscles between the groups indicated no significant differences (p>0.05). The study of attenuation for muscle adipose tissue indicated no significant difference in the values of between the OA-Young and Aged-S groups for all the muscles evaluated. Conclusion: Young individuals with knee OA present muscle morphological characteristics similar to those found during the aging process, characterizing early aging of the thigh muscles. RESUMEN Introducción: La artrosis (OA) es una enfermedad inflamatoria degenerativa crónica, caracterizada por la degeneración progresiva del cartílago articular, que afecta principalmente a la articulación de la rodilla. La población anciana es la más afectada, intensificándose el proceso de envejecimiento. La concentración de tejido adiposo intramuscular en los músculos del muslo y la OA de rodilla a diferentes edades sigue siendo objeto de investigación. Objetivo: Investigar el área transversal de los músculos del muslo a diferentes edades con la relación de la presencia de grasa intramuscular y la OA de rodilla. Métodos: Se emparejó a 80 participantes en 4 grupos: Grupo de jóvenes con osteoartritis y Grupo de mayores con osteoartritis, ambos sedentarios y con diagnóstico de OA de rodilla II o III; Grupo de jóvenes sanos y Grupo de mayores sanos, ambos sanos y sedentarios. Los grupos fueron emparejados según sexo e índice de masa corporal, sometidos a evaluación fisioterapéutica, cuestionario WOMAC, exámenes radiográficos de las rodillas y tomografía computarizada del muslo. El análisis morfométrico se realizó manualmente con el software ITK-SNAP (versión 3.6), por un único evaluador. En el análisis estadístico se utilizó la prueba ANOVA unidireccional seguida del post-hoc de Bonferroni para las variables dependientes (p≤0,05). Resultados: La comparación del área transversal de los músculos recto femoral, vasto lateral, semitendinoso, sartorio y gracilis entre los grupos no indicó diferencias significativas (p>0,05). El estudio de la atenuación para el tejido adiposo muscular no indicó diferencias significativas en los valores de entre los grupos OA-Young y Aged-S para todos los músculos evaluados. Conclusión: Individuos jóvenes con OA de rodilla presentan características morfológicas musculares similares a las encontradas durante el proceso de envejecimiento, caracterizando el envejecimiento precoz de los músculos del muslo. A população idosa é a mais atingida, intensificando o processo de envelhecimento. A concentração de tecido adiposo intramuscular nos músculos da coxa e a OA de joelho em diferentes idades permanece em investigação. Objetivo: Investigar a área de secção transversa dos músculos da coxa em diferentes idades com a relação da presença de gordura intramuscular e OA de joelho. Métodos: 80 participantes foram pareados em 4 grupos: Grupo Osteoartrite Jovem e Grupo Osteoartrite Idoso, ambos sedentários com diagnóstico de OA de joelho II ou III; Grupo Jovem Saudável e Grupo Idoso Saudável, ambos saudáveis e sedentários. Os grupos foram pareados de acordo com sexo e índice de massa corporal, submetidos à avaliação fisioterapêutica, questionário WOMAC, exames de raio-x dos joelhos e tomografia computadorizada da coxa. A análise morfométrica foi realizada manualmente com software ITK-SNAP (versão 3.6), por um único avaliador. Análise estatística utilizou o teste ANOVA one-way seguido pelo post-hoc de Bonferroni, para variáveis dependentes (p≤0,05). Resultados: A comparação da área de secção transversa dos músculos reto femoral, vasto lateral, semitendinoso, sartório e grácil, entre os grupos, não indicou diferenças significativas (p>0,05). O estudo da atenuação para o tecido adiposo muscular não indicou diferença significativa nos valores de entre os grupos OA-Jovem e Idoso-S para todos os músculos avaliados. Conclusão: Indivíduos jovens com OA de joelho apresentam características morfológicas musculares semelhantes às encontradas durante o processo de envelhecimento, caracterizando um envelhecimento precoce dos músculos da coxa.Nível de Evidência III; Estudo retrospectivo transversal e observacional.

La osteoartritis (OA) de la articulación temporomandibular (ATM) es un desorden degenerativo de etiología multifactorial, que requiere un manejo interdisciplinario. Es considerada como la enfermedad degenerativa más frecuente de la articulación. Por esto es importante conocer lo más preciso posible las estructuras internas del área donde se requiere realizar la intervención o tratamiento, en este caso la ATM. Para esto existen distintos exámenes radiográficos complementarios como: tomografía computarizada, resonancia magnética y por último la tomografía computarizada de haz cónico (CBCT), debido a su capacidad de visualizar tridimensionalmente y con buena definición las estructuras óseas y distintas patologías o alteraciones presentes. A pesar de esto, no hay suficiente evidencia actual que demuestre la frecuencia de signos óseos de osteoartritis presentes en ATM según edad y sexo en la población Chilena. El objetivo este trabajo consistió en Determinar frecuencia de los signos óseos en osteoartritis de ATM mediante CBCT en una población adulta Chilena atendida en un centro radiológico durante los años 2021-2022. Se realizó un estudio observacional descriptivo, donde se observó informes radiológicos de CBCT en pacientes adultos atendidos en el centro radiológico privado de Valdivia durante el periodo del primer semestre del 2021 a primer semestre 2022. Se evaluó la presencia de los siguientes signos óseos imagenológicos: aplanamiento de superficie articular, erosión superficial, osteofitos condilares, esclerosis subcondral, quistes subcortical, esclerosis generalizada, cuerpos libres intraarticulares, reabsorción completa y parcial de la cabeza condilar y trabeculado heterogéneo. De un total de 101 exámenes, 70 exámenes fueron considerados válidos para este estudio según los criterios de selección. Los 31 exámenes restantes no calificaron según los criterios o no presentaban osteoartritis de ATM. De los 70 pacientes 58 pertenecían a mujeres y 12 a hombres. El promedio de edad fue de 37,2 años. Los signos imagenológicos más frecuentes fueron: Trabeculado heterogéneo, Aplanamiento de la superficie articular, Esclerosis subcondral condilar.


Osteoarthritis (OA) of the temporomandibular joint (TMJ) is a degenerative disorder of multifactorial etiology, requiring interdisciplinary management. It is considered the most common degenerative joint disease. For this reason, it is important to know asprecisely as possible the internal structures of the area where the intervention or treatment is required, in this case the TMJ. For this purpose, there are several additional radiographic examinations such as: computed tomography, magnetic resonance imaging and finally cone-beam computed tomography (CBCT), due to its ability to visualize three-dimensionally and with good definition the bone structures and different pathologies or present alterations. Despite this, there is not enough current evidence to demonstrate the frequency of osteoarthritis bone signs present in TMJ according to age and gender in the Chilean population. The aim of this study was to determine the frequency of bone signs of TMJ osteoarthritis through CBCT in a Chilean adult population attended in a radiological center during 2021-2022. A descriptive observational study was made, where CBCT radiological reports were observed in adult patients attended in a private radiological center in Valdivia during the first semester of 2021 to the first semester of 2022. The presence of the following imaging bone signs was evaluated: flattening of the articular surface, surface erosion, condylar osteophytes, subchondral sclerosis, subcortical cysts, generalized sclerosis, intraarticular free bodies, complete and partial reabsorption of the condylar head and heterogeneous trabeculate. Of a total of 101 examinations, 70 examinations were considered valid for this study according to the selection criteria. The remaining 31 examinations did not qualify according to the criteria or did not present TMJ osteoarthritis. Of the 70 patients, 58 were female and 12 were male. The average age was 37.2 years. The most frequent imaging signs were: heterogeneous trabeculation, flattening of the articular surface, subchondral condylar sclerosis.

De todas las enfermedades del sistema musculoesquelético, la osteoartritis (OA) es la más frecuente de las artritis, lo que conlleva a enormes gastos para la sociedad y una gran morbilidad para las personas que la padecen.  Se caracteriza por afectar toda la articulación, incluyendo la degradación del cartílago, el remodelamiento óseo, la formación de osteofitos y la inflamación de la membrana sinovial. En consecuencia, se produce dolor, rigidez, hinchazón y pérdida de la función articular con marcada limitación funcional. El manejo de esta entidad implica una terapia no farmacológica, el uso de drogas para el alivio del dolor y cirugía cuando las medidas anteriores han fallado. Dentro de las medidas no farmacológicas destacan la educación, los ejercicios para el fortalecimiento muscular, la terapia física, la disminución de peso y las ortesis o dispositivos para asistir en la deambulación. En cuanto a las drogas recomendadas podemos mencionar a los antiinflamatorios no esteroideos tanto tópicos como orales y los glucocorticoides intraarticulares. Finalmente, la meniscectomía parcial por artroscopía solo se ha recomendado cuando existe bloqueo en la rodilla. En aquellos individuos que sufran de un dolor insoportable o una limitación funcional severa por una enfermedad avanzada debe considerarse el reemplazo total de la articulación. Of all the diseases of the musculoskeletal system, osteoarthritis (OA) is the most common arthritis, leading to enormous costs to society and great morbidity for sufferers.  It is characterized by involvement of the entire joint, including cartilage degradation, bone remodeling, osteophyte formation and inflammation of the synovial membrane. This results in pain, stiffness, swelling and loss of joint function with marked functional limitation. Management of this entity involves non-pharmacological therapy, the use of drugs for pain relief and surgery when previous measures have failed. Non-pharmacological measures include education, muscle strengthening exercises, physical therapy, weight reduction and orthoses or devices to assist ambulation. As for recommended drugs, non-steroidal anti-inflammatory drugs, both topical and oral, and intra-articular glucocorticoids are recommended. Finally, partial meniscectomy by arthroscopy has only been recommended when there is knee locking. In individuals suffering from excruciating pain or severe functional limitation due to advanced disease, total joint replacement should be considered. (provided by Infomedic International)

Introdução:A osteoartrite é uma doença degenerativa caracterizada pela deterioração progressiva da cartilagem articular, resultando em dor e incapacidade articular total em estágios avançados.Éconsiderada um dos distúrbios articulares mais comuns em todo o mundo e sua prevalência está aumentando constantemente devido ao envelhecimento, dietas inflamatórias e inatividade física. Objetivo:Investigar a contribuição da microbiota intestinal e dos componentes dietéticos, naperspectiva dediminuir as patologias associadas à osteoartrite. Metodologia:Trata-se de uma revisão integrativa desenvolvida a partir da seleção de artigos disponíveis escritos nalíngua inglesa, publicados nas bases de dados Pubmed e Science Direct. Resultados:No total, 25.583 artigos foram encontrados na busca, após os critérios de exclusão, 19 artigos compuseram o corpo de análise da revisão. Pesquisas em animais mostram que os efeitos induzidos por dieta rica em gordura foram evidentes e indicaram uma inflamação sistêmica de baixo grau resultando no agravamento da oesteoartritepor meio do aumento da degeneração da cartilagem. Dado ao impacto potencial da dieta na oesteoartrite, foram realizados estudos para avaliar a dieta mediterrânea, os níveis de ômega 3 e 6, vitamina C e E, com destaque para a oligofrutose, uma abordagem nova para tratar a oesteoartriteda obesidade. Conclusões:Conclui-se que apesar de já existir alguma evidência da utilidade da nutrição por meioda dieta alimentar como complemento da terapêutica na osteoartrite são necessários mais estudos que comprovem as intervenções na redução máxima dos marcadores inflamatórios ocasionando o alíviodos sintomas em pacientes com oesteoartrite (AU).


Introduction:Osteoarthritis is a degenerative disease characterized by progressive deterioration of the articular cartilage, resulting in pain and total joint disability in advanced stages. It is considered one of the most common joint disorders worldwide and its prevalence is steadily increasing due to aging, inflammatory diets and physical inactivity. Objective:The aim of this literature review was to investigate the contribution of intestinal microbiota and dietary components to try to reduce the pathologies associated with osteoarthritis.Methodology:This is an integrative review, developed from the selectionof available articles written in English, published in the Pubmed and Science Direct databases. Results:Intotal, 25.583articleswerefoundinthesearch,aftertheexclusioncriteria,19 articles madeupthebodyofanalysisofthereview.Animal research shows that the effects induced by a high-fat diet were evident and indicated low-grade systemic inflammation resulting in worsening osteoarthritis by increasing cartilage degeneration. Given the potential impact of diet on osteoarthritis, studies have been conducted to evaluate the Mediterranean diet, omega 3 and 6 levels, vitamin C and E, especially oligofructose, a new approach to treat obesity osteoarthritis.Conclusions:It is concluded that although there is already some evidence of the usefulness of nutrition through the diet as a complement to therapy in osteoarthritis, further studiesare needed to prove the interventions in the maximum reduction of inflammatory markers will cause the relief of symptoms in patients with osteoarthritis (AU).


Introducción: La artrosis es una enfermedad degenerativa caracterizada por el deterioro progresivo del cartílago articular, que se traduce en dolor e incapacidad articular total en estadios avanzados. Se considera uno de los trastornos articulares más comunes en todo el mundo y su prevalencia aumenta constantemente debido al envejecimiento, las dietas inflamatorias y la inactividad física. Objetivo: El objetivo de esta revisión fue investigar la contribución de la microbiota intestinal y los componentes de la dieta, en un intento por reducirlas patologías asociadas a la artrosis. Metodología: Se trata de una revisión integradora, desarrollada a partir de la selección de artículos disponibles escritos en inglés, publicados en las bases de datos Pubmed y Science Direct. Resultados: En total, se encontraron 25.583 artículos en la búsqueda, después de los criterios de exclusión, 19 artículos conformaron el cuerpo de análisis de la revisión.La investigación en animales muestra que los efectos inducidos por una dieta alta en grasas fueron evidentes e indicaron una inflamación sistémica de bajo grado que resultó en un empeoramiento de la osteoartritis a través de una mayor degeneración del cartílago. Dado el impacto potencial de la dieta en la osteoartritis, se han realizado estudios para evaluar ladieta mediterránea, los niveles de omega 3 y 6, vitamina C y E, con énfasis en la oligofructosa, un nuevo enfoque para tratar la osteoartritis por obesidade.Conclusiones: Se concluye que aunque ya existe alguna evidencia de la utilidad de la nutrición a través de la dieta como complemento al tratamiento de la artrosis, son necesarios más estudios que prueben intervenciones en la reducción máxima de los marcadores inflamatorios, provocando el alivio de los síntomas en pacientes con osteoartritis (AU).

Background: Prolotherapy may be a good option in the complementary treatment of knee osteoarthritis (OA) patients, specifically for the increase of functionality. Objective: To determine the effectiveness of prolotherapy in OA grade II - III in the functionality and muscular work of knee flexors and extensors. Material and methods: a double-blind randomized controlled clinical trial was conducted. It included patients diagnosed with OA grades II-III. The experimental group was infiltrated in both knees with 6 ml of 25% glucose solution and 0.05% lidocaine; control group with 0.45% saline solution and 0.05% lidocaine. All patients received a comprehensive rehabilitation program. The isokinetic work of the knee flexor and extensor muscles, pain and functionality were measured, prior to infiltration and at 3-month follow-up. To compare the difference in means, the Student's T test was applied, considering P<0.05 as significant. The project was approved by the local ethics and research committee. Results: 37 patients were recruited, 17 in the prolotherapy group. There were no intergroup differences in functionality, isokinetic knee flexor/extensor work, and pain at baseline, or during follow-up up to 12 weeks. Conclusions: In the present study we identify that both prolotherapy and saline infiltration increased functionality, strength, and decreased pain; however, no statistically significant difference was found between the two group.


Introducción: la proloterapia puede ser buena opción en el tratamiento complementario de pacientes con osteoartrosis de rodilla (OA), específicamente para el incremento de la funcionalidad. Objetivo: determinar la efectividad de la proloterapia en OA grado II-III en la funcionalidad y en el trabajo muscular de flexores y extensores de rodilla. Material y métodos: se realizó un ensayo clínico controlado aleatorizado a doble ciego. Incluyó a pacientes con el diagnostico de OA grados II-III. Al grupo experimental se les infiltraron 6 mL de solución glucosada al 25% y lidocaína al 0.05% en ambas rodillas; al grupo control con solución salina al 0.45% y lidocaína al 0.05%. Todos los pacientes recibieron un programa integral de rehabilitación. Se midió el trabajo isocinético de los músculos flexores y extensores de rodilla, dolor y funcionalidad, previa infiltración y a los tres meses de seguimiento. Para comparar la diferencia de medias, se aplicó la prueba de t de Student, considerando p < 0.05 como significativo. El proyecto se aprobó en el comité local de ética e investigación. Resultados: se reclutaron 37 pacientes, 17 en el grupo de proloterapia. No hubo diferencias intergrupo en la funcionalidad, trabajo isocinético de flexores/extensores de rodilla y dolor iniciales, ni durante el seguimiento hasta las 12 semanas. Conclusiones: en el presente estudio identificó que tanto la proloterapia como la infiltración con solución salina incrementaron la funcionalidad, fuerza y disminuyeron el dolor; sin embargo, no se encontró diferencia estadísticamente significativa entre los dos grupos.

Antecedentes y objetivos: La osteoartritis de la base del primer metacarpiano (OA CMC-1) es una patología que afecta mayormente a mujeres posmenopáusicas. Los síntomas principales que acompañan esta patología son el dolor, la pérdida de fuerza y la disminución de capacidad motora fina. A pesar de que se ha demostrado un déficit propioceptivo en personas con OA CMC-1, no hay evidencia suficiente sobre los efectos del trabajo propioceptivo. El objetivo de este estudio es determinar la efectividad del entrenamiento propioceptivo en la recuperación funcional. Materiales y métodos: Se incluyeron un total de 57 pacientes, 29 del grupo control y 28 en el experimental. Ambos grupos llevaron a cabo el programa de intervención, y al grupo experimental se le incluyó además un protocolo de trabajo propioceptivo. Las variables de estudio fueron el dolor (EVA), percepción del desempeño ocupacional (COMP), sensación de posición (SP) y sensación de fuerza (SF). Resultados: Se observó una mejora estadísticamente significativa en el grupo experimental en el dolor (p<0,05) y el desempeño ocupacional (p<0,001) a los 3 meses de tratamiento. No se encontraron diferencias estadísticas en la sensación de posición (SP) y la sensación de fuerza (FS). Discusión y conclusiones: La correlación de nuestros resultados y los de estudios precedentes en cuanto a la efectividad del entrenamiento propioceptivo sobre las variables de estudio nos permite afirmar que la incorporación de un protocolo de ejercicios propioceptivos disminuye el dolor y mejora notablemente el desempeño ocupacional. Although a proprioceptive deficit has already been demonstrated in people with CMC-1 OA, there is insufficient evidence regarding the effects of proprioceptive training. The main objective of this study is to determine the effectiveness of proprioceptive training in functional recovery. Materials and methods: A total of 57 patients were included in the study, 29 in the control group and 28 in the experimental group. Both groups underwent the same basic intervention programme, but the experimental group included a proprioceptive training protocol. Variables of the study were pain (VAS), perception of occupational performance (COMP), sense position (SP) and force sensation (FS). Results: Statistically significant improvement was observed in the experimental group in pain (p<.05) and occupational performance (p<.001) after 3 months of treatment. No statistical differences were found in sense position (SP) or sensation of force (FS). Discussion and conclusions: The results concord with previous studies focussing on proprioception training. The incorporation of a proprioceptive exercise protocol reduces pain and significantly improves occupational performance. (AU)

Introducción: la osteoartritis es una enfermedad crónica y progresiva que afecta a más del 30 % de las personas mayores de 60 años. Actualmente, se reconoce la osteoartritis como una enfermedad multifactorial. Se emplean varios tratamientos conservadores en el manejo de la osteoartritis de rodilla (AINE, analgésicos y terapias intraarticulares). Se realizó un ensayo clínico aleatorizado para determinar si una terapia a base de 10 g de colágeno hidrolizado y 100 mg de fucoidano (Hydroidan pro, Acten, Suiza) era más efectiva que las terapias intraarticulares. Métodos: se dividió a los pacientes en 3 grupos. El primero recibió 23 g de ACTEN® cada día durante 3 meses. Los otros grupos recibieron una única inyección intraarticular de ácido hialurónico (5 ml) o plasma rico en plaquetas (3 ml). Se emplearon las escalas WOMAC, SF-12 y VAS para valorar el dolor al inicio, 4, 12 y 24 semanas después. Resultados: se incluyó a 108 pacientes con osteoartritis de rodilla de grado II-III que participaron en un estudio de seguimiento de 24 semanas de duración. La edad media fue de 57 años (53-65). Los 3 grupos rebajaron la puntuación en el grupo WOMAC (p < 0,001). El grupo que recibió colágeno y fucoidano obtuvo puntuaciones más bajas en las escalas WOMAC y VAS que los grupos que recibieron ácido hialurónico y plasma rico en plaquetas a las 24 semanas (p < 0,001). Conclusiones: el colágeno y el fucoidano tomado por vía oral, a diario, y durante 12 semanas parecen cosechar mejores resultados en las escalas WOMAC y VAS que las terapias intraarticulares a base de ácido hialurónico o plasma rico en plaquetas. Se debería de intentarse combinar terapias orales e intraarticulares para determinar su perfil de eficacia.(AU)


Introduction: osteoarthritis is a chronic and progressive disease. It affects over 30 % of people older than 60. Osteoarthri-tis is currently recognized as a multifactorial disease. Various conservative treatments are used in the management of kneeosteoarthritis (NSAIDs, analgesics, and intra-articular therapy). We conducted a randomized clinical trial to determine if a10 g therapy of hydrolyzed collagen along with 100 mg fucoidan (Hydroidan pro, Acten, Switzerland) is more effective thanintra-articular therapies.Methods: we divided patients into 3 groups. The first group received 23 g of ACTEN®, daily, for 3 months. The other groupsreceived a single intra-articular injection of hyaluronic acid (5 ml) or platelet-rich plasma (3 ml). We used the WOMAC scale,the SF-12 scale, and the VAS for pain at baseline, and 4, 12, and 24 weeks later.Results: we enrolled 108 patients with grade II-III knee osteoarthritis who underwent a 24-week follow-up study. The meanage was 57 years (53-65). The three groups showed low scores in the WOMAC group (p < 0.001). The collagen with fucoid-an group had lower WOMAC and VAS scores compared with the hyaluronic acid and platelet-rich plasma groups at 24weeks (p < 0.001).Conclusions: collagen along with fucoidan taken orally, daily, for 12 weeks seem to have better results in the WOMACand VAS scales compared with intra-articular therapies such as hyaluronic acid or platelet-rich plasma. Combined oral andintra-articular therapies should be tried to determine their efficacy profile.(AU)

INTRODUCTION: Malnutrition in elderly patients with hip fracture has been described as a factor associated with poor outcomes. This evidence for elderly patients undergoing elective total hip arthroplasty (THA) for osteoarthritis is not well-established. METHODS: We retrospectively studied a cohort of patients with an age ≥65 years admitted to the hospital for THA between January 2018 and December 2020. Demographic characteristics were collected. Albumin and total lymphocyte count were included in the pre-operative analysis and 24h postoperatively. Nutritional screening was carried out 24h postoperatively. GLIM criteria were applied for the diagnosis of malnutrition. RESULTS: 25 patients out of the total cohort of 65 (38.4%) were malnourished. Five out of six patients (83.3%) with postoperative complications presented malnutrition compared with 20 of 59 patients (33.8%) without postoperative complications (P=0.028). Mean length of stay (LOS) was 3.49±0.88 days. Five out of six (83.3%) patients with postoperative complications presented LOS >3 days compared with one out of six patients (16.6%) with LOS ≤3 days (P=0.009). 26 patients (40%) were referred to a community health centre, no differences in diagnosis of malnutrition were detected compared with patients who were discharged home. Patients with a diagnosis of malnutrition were significantly older, had a lower body mass index (BMI), a lower preoperative and postoperative albumin and worse anthropometric parameters. CONCLUSIONS: Malnutrition is associated with postoperative complications and longer LOS in elderly patients with elective THA. Risk factors for malnutrition are older age and low BMI.

Developmental dysplasia of the hip is the cause of approximately one third of secondary coxarthrosis. Anatomy alterations make it difficult to place a total hip prosthesis in its anatomical position and for it to be stable in the long term; there are several techniques to achieve this goal. In the present work, we used autograft of the femoral head (shelf graft or reinforced roof), to improve the coverage of the acetabular component with favorable results. MATERIALS AD METHODS: Sixteen cases were included in 14 patients with a diagnosis of developmental dysplasia of the hip (13 women and one man), the mean age was 44.3 years (range 35-68 years), with a mean follow-up of 7 years (range 1-15 years). All the cases were evaluated clinically and radiographically, to demonstrate the osseointegration of the graft and the functional results in the medium term. RESULTS: All the acetabular components were placed in anatomical position (Ranawat technique), the mean percentage of host bone coverage was 54.53% (range 43.28-79.05%), obtaining additional coverage with the bone graft of 45.13%. Osseointegration of the graft of 100% at 12 weeks, resorption of the graft from the sixth month, stabilizing in the third postoperative year. Only one case of dislocation is reported, no cases of infection, loosening, heterotopic ossification or revision. CONCLUSION: This procedure has shown good functional results in the medium term with 100% osseointegration, despite cases of severe bone resorption of the graft that does not compromise the stability of the prosthesis.

Introducción: El bienestar del adulto mayor constituye una prioridad del Sistema Sanitario, por lo que es necesario contar con instrumentos válidos y confiables para su evaluación. Objetivo: Validar la Escala de Bienestar Psicológico de Carol Ryff, en una muestra de adultos mayores con diagnóstico presuntivo de osteoartrosis de rodilla, de la provincia de Villa Clara, Cuba. Métodos: Se asumió el paradigma de investigación cuantitativo y un tipo de estudio instrumental. La selección muestral fue probabilística, conformada por 170 adultos mayores. Se aplicó el instrumento Escala de Bienestar Psicológico de Carol Ryff (42 aspectos); para el procesamiento de la información se emplearon las pruebas estadísticas: coeficiente alpha de Cronbach y el Análisis Factorial Exploratorio. Resultados: Se determinó una estructura coherente de 5 factores: autoaceptación, propósito en la vida, dominio del entorno, autonomía y relaciones positivas con los demás. Se apreció una elevada consistencia interna de la escala global y sus dimensiones, así como adecuadas correlaciones entre los diversos aspectos. Conclusión: El instrumento posee adecuadas propiedades psicométricas y permite evaluar el constructo en esta población.


Introduction: the well-being of the elderly is a priority of the health system, that is why valid and reliable instruments are necessary for its evaluation. Objective: to validate the Carol Ryff's Psychological Well-Being Scale in a sample of older adults from Villa Clara province, Cuba with a presumptive diagnosis of knee osteoarthritis. Methods: a quantitative and instrumental research was assumed. The sample selection was probabilistic, made up of 170 older adults with this condition. The Carol Ryff's Psychological Well-Being Scale was applied (42 items); statistical tests were used for information processing such as Cronbach's alpha coefficient and Exploratory Factor Analysis. Results: a coherent factorial structure of 5 factors was determined: self-acceptance, purpose in life, environmental mastery, autonomy, and positive relationships with others. A high internal consistency of the global scale and its dimensions were observed, as well as adequate inter-item correlations. Conclusion: the instrument has adequate psychometric properties and allows us to evaluate the construct in this population.

Fundamento el incremento de la esperanza de vida viene aparejado con la necesidad del adulto mayor de desenvolverse en su medio y garantizar una longevidad plena. Objetivo determinar la efectividad de la peloideterapia en adultos mayores con osteoartrosis de rodilla. Métodos se realizó un estudio de intervención, cuasi experimental, en 30 pacientes con osteoartrosis de rodilla, seleccionados mediante aleatorización simple, quienes fueron atendidos en el área de salud de Yaguaramas (Abreus, Cienfuegos), en el período comprendido entre marzo y junio de 2021. Se realizó una encuesta general. La evolución del dolor se determinó mediante la escala de Likert, y la limitación articular se analizó mediante la medición del arco articular de la rodilla con el goniómetro. Resultados predominaron los pacientes del grupo de edades de 66-70 años y del sexo femenino; con tiempo de evolución entre 5-10 años (60 %); y con ambas rodillas afectadas (56,7%). Los principales signos y síntomas fueron el dolor (100 %) y la limitación del movimiento articular (93,3 %), los cuales disminuyeron tras el tratamiento. El 53,3 % refirió dolor ligero antes del tratamiento; después del tratamiento el 83,3 % describió ausencia de dolor. El 76,7 % de los pacientes manifestó limitación en la flexión de la rodilla, que después del tratamiento disminuyó a 13,3 %. Conclusiones la peloideterapia influye positivamente en los adultos mayores con osteoartrosis de rodilla, al mejorar su estado funcional e independencia personal, además de reducir las demandas de servicios médicos.


Background the increase in life expectancy is coupled with the elderly's need to function in its media and guarantee full longevity. Objective to determine the effectiveness of peloidtherapy in older adults with osteoarthritis of the knee. Method a quasi-experimental intervention study was carried out in 30 patients with osteoarthritis of the knee, selected by simple randomization, who were treated in the health area of Yaguaramas (Abreus, Cienfuegos), between March and June of 2021. A general survey was carried out. The evolution of pain was determined using the Likert scale, and joint limitation was analyzed by measuring the knee joint arc with the goniometer. Results there was a predominance of patients in the age group of 66-70 years old and of the female sex; with evolution time between 5-10 years (60%); and with both knees affected (56.7%). The main signs and symptoms were pain (100%) and limitation of joint movement (93.3%), which decreased after treatment. 53.3% reported light pain before treatment; after treatment, 83.3% described absence of pain. 76.7% of the patients manifested limitation in knee flexion, which after treatment decreased to 13.3%. Conclusions peloidtherapy has a positive influence on older adults with osteoarthritis of the knee, by improving their functional status and personal independence, as well as reducing the demands for medical services.

Se realizó un estudio cuasiexperimental prospectivo para evaluar la utilidad de la laserterapia en el tratamiento de la gonartrosis, en el Servicio de Rehabilitación Integral del Policlínico Docente «Camilo Cienfuegos», de Zuluetas, Villa Clara. De un universo de 76 pacientes diagnosticados por Ortopedia con gonartrosis, se tomó una muestra de 60, distribuidos al azar en dos grupos equitativos, el grupo de estudio recibió tratamiento con laserterapia y el grupo control con calor infrarrojo; se evaluó la intensidad del dolor según la escala visual, y se practicó un examen físico que incluía la medición de los ángulos de movimiento de la articulación de la rodilla y su capacidad funcional. Se realizó un segundo examen para evaluar su utilidad. Se comprobó que ambas modalidades terapéuticas aportaron beneficios, con resultados significativamente superiores en los pacientes atendidos con laserterapia.


A prospective, quasi-experimental study was carried out to evaluate the usefulness of laser therapy in the treatment of gonarthrosis, in the comprehensive rehabilitation service at «Camilo Cienfuegos» Teaching Polyclinic in Zuluetas, Villa Clara. A sample of 60 patients randomly distributed into two equitable groups was taken from a universe of 76 patients diagnosed with gonarthrosis in the Orthopaedic service, the study group received treatment with laser therapy and the control ones with infrared heat; pain intensity was evaluated according to the visual scale and a physical examination was performed including measurement of the knee joint angles and its functional capacity. A second exam was conducted to assess its usefulness. We found that both therapeutic modalities have benefits in the treatment of gonarthrosis, with significantly better results in patients treated with laser therapy.

INTRODUCTION AND OBJECTIVES: To determine the disease burden and costs in patients with hip or knee OA and chronic moderate-to-severe refractory pain, receiving strong opioids in Spain. MATERIALS AND METHODS: This was a 36-month longitudinal secondary analysis of the real-word OPIOIDS study. Patients aged ≥18 years with hip or knee OA and chronic moderate-to-severe refractory pain receiving strong opioids were considered. The disease burden included analgesia assessments (NRS scale), cognitive functioning (MMSE scale), basic activities of daily living (Barthel index), and comorbidities (severity and frequency). Costs due to the use of healthcare resources and productivity loss were estimated. RESULTS: 2832 patients were analyzed; age was 72.0 years (SD=14.3), 76.8% were women. Patients had mainly been treated with fentanyl (n=979; 37.6%), tapentadol (n=625; 24.0%), oxycodone (n=572; 22.0%), and buprenorphine (n=425; 16.3%). Pain intensity decreased by 1 point (13.7%), with a 2.6-point decline in the cognitive scale (14.3%, with a 5.3%-increase in patients with cognitive deficit) over a mean treatment period of 384.6 days (SD: 378.8). Barthel scores decreased significantly yielding to a slightly increase in proportion of patients with severe-to-total dependency; 1.2%-2.9%. In the first year of treatment, average healthcare costs were €2013/patient, whereas the average productivity loss cost was €12,227/working-active patient. DISCUSSION AND CONCLUSIONS: Strong opioids resulted in high healthcare costs with a limited reduction in pain, an increase in cognitive deficit, and a slight increase of patients with severe to total dependency over 36 months of treatment.

Introduction and objectives: To determine the disease burden and costs in patients with hip or knee OA and chronic moderate-to-severe refractory pain, receiving strong opioids in Spain. Materials and methods: This was a 36-month longitudinal secondary analysis of the real-word OPIOIDS study. Patients aged ≥18 years with hip or knee OA and chronic moderate-to-severe refractory pain receiving strong opioids were considered. The disease burden included analgesia assessments (NRS scale), cognitive functioning (MMSE scale), basic activities of daily living (Barthel index), and comorbidities (severity and frequency). Costs due to the use of healthcare resources and productivity loss were estimated. Results: 2832 patients were analyzed; age was 72.0 years (SD=14.3), 76.8% were women. Patients had mainly been treated with fentanyl (n=979; 37.6%), tapentadol (n=625; 24.0%), oxycodone (n=572; 22.0%), and buprenorphine (n=425; 16.3%). Pain intensity decreased by 1 point (13.7%), with a 2.6-point decline in the cognitive scale (14.3%, with a 5.3%-increase in patients with cognitive deficit) over a mean treatment period of 384.6 days (SD: 378.8). Barthel scores decreased significantly yielding to a slightly increase in proportion of patients with severe-to-total dependency; 1.2%–2.9%. In the first year of treatment, average healthcare costs were €2013/patient, whereas the average productivity loss cost was €12,227/working-active patient. Discussion and conclusions: Strong opioids resulted in high healthcare costs with a limited reduction in pain, an increase in cognitive deficit, and a slight increase of patients with severe to total dependency over 36 months of treatment.(AU)


Introducción y objetivos: Determinar la carga de la enfermedad y los costes en pacientes con osteoartritis de cadera y rodilla y dolor crónico refractario moderado-severo, en tratamiento con opioides mayores en España. Materiales y métodos: Se trata de un subanálisis de 36 meses de duración, procedente del estudio observacional OPIOIDS. Participaron pacientes con una edad ≥18 años, diagnosticados con osteoartritis de cadera y rodilla y dolor crónico refractario moderado-severo, en tratamiento con opioides mayores. La carga de la enfermedad incluyó la evaluación de la analgesia (escala NRS), del funcionamiento cognitivo (escala MMSE), de la capacidad para realizar las actividades de la vida diaria (índice de Barthel) y de las comorbilidades (gravedad y frecuencia). También se estimaron los costes asociados al uso de recursos sanitarios y a la productividad laboral. Resultados: Se analizaron 2.832 pacientes (edad: 72,0 años [DE: 14,3]; mujeres: 76,8%), que habían sido principalmente tratados con fentanilo (n=979; 37,6%), tapentadol (n=625; 24,0%), oxicodona (n=572; 22,0%) y buprenorfina (n=425; 16,3%). La intensidad del dolor disminuyó una unidad (13,7%), con una reducción de 2,6 unidades en la escala cognitiva (14,3% y aumento del 5,3% en los pacientes con déficit cognitivo) durante una media de 384,6 días (DE: 378,8). Las puntuaciones en la escala de Barthel disminuyeron significativamente, con un ligero aumento en la proporción de pacientes con dependencia grave/total, entre 1,2% y 2,9%. En el primer año, los costes sanitarios medios fueron 2.013€/paciente, mientras que los costes medios de pérdida de productividad fueron 12.227€/trabajador. Discusión y conclusiones: El tratamiento con opioides mayores durante 36 meses implicó elevados costes sanitarios, con una eficacia analgésica limitada, un aumento del déficit cognitivo y un ligero aumento de los pacientes con dependencia grave/total.(AU)
